Communicable Disease Reporting
Many communicable and chronic diseases are required by California state law to be reported to the Marin County
Department of Health and Human Services, Communicable Disease Control. If you are a health care provider or laboratory professional
and diagnose or identify probable or suspect cases of reportable disease in the State of California,
please report to us by phone, 415-473-7805 (English) or 415-507-2623 (Spanish), or fax a Confidential Morbidity Form (CMR) (415-473-6002).
For diseases that require immediate response and occur before 9:00am or after 5:00 pm, Monday-Friday or on
a weekend or holiday, please call the Marin County Communications Center (415-499-9464), and ask for the on-duty public health officer.

Public Health Officer
The Public Health Officer (PHO) has a leadership role across
a wide range of public health functions and activities, including preparedness
and response efforts related to bioterrorism, communicable disease occurrence
and outbreaks, and other public health emergencies. Serving on the strategic
management team in the Division of Public Health Services, the PHO provides
leadership regarding clinical/medical and public health policy issues affecting
the health status of the community, provides medical and public health direction
to communicable disease programs, and carries out statutory responsibilities
