Communicable Disease Reporting

Many communicable and chronic diseases are required by California state law to be reported to the Marin County Department of Health and Human Services, Communicable Disease Control. If you are a health care provider or laboratory professional and diagnose or identify probable or suspect cases of reportable disease in the State of California, please report to us by phone, 415-473-7805 (English) or 415-507-2623 (Spanish), or fax a Confidential Morbidity Form (CMR) (415-473-6002).

For diseases that require immediate response and occur before 9:00am or after 5:00 pm, Monday-Friday or on a weekend or holiday, please call the Marin County Communications Center (415-499-9464), and ask for the on-duty public health officer.
